Patients are registered with the practice as a whole and not with a named doctor.
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.
Please note that if you do not submit all of the information within 24 hours the registration will reset and you will need to start again.
