Register as a new patient
All patient registrations are now required to be completed online. Please complete all three online forms below and then email a picture of your proof of address and ID to
swlccg.brunswick.surgery@nhs.net
Please note we can only accept patients who reside within the practice boundary shown.
Patients are registered with the practice as a whole and not with a named doctor. Nevertheless, you do have the right to express a preference as to which GP you would like to see or speak to.
Registrations are usually completed within 3 days of receipt. If neccessary downloadable forms can be found below.

Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.You cannot register as a temporary patient at a practice in the town or area where you are already registered.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Simply send it back to us via: swlccg.brunswick.surgery@nhs.net along with your photo ID so we can get you registered and book you an appointment with a GP.
